MELD-Na score (liver disease)

Predicts 3-month mortality in cirrhosis. Used for liver transplant prioritization. Anesthesia note: MELD ≥ 15 = significant perioperative risk; ≥ 20 = high-risk for non-transplant surgery.

MELD = 10 × (0.957 × ln(creat) + 0.378 × ln(bili) + 1.12 × ln(INR) + 0.643)

Floors: bili ≥ 1, creat ≥ 1 (≤ 4), INR ≥ 1

MELD-Na adds Na correction if MELD greater than 11:
MELD-Na = MELD + 1.32 × (137 − Na) − 0.033 × MELD × (137 − Na)
