Child-Pugh score
Anesthesia risk goes up sharply at Child-Pugh B/C. Drug PK alters: avoid hepatically-cleared drugs at high dose, expect prolonged neuromuscular blockade, watch for coagulopathy + portal HTN.

Sum 5 components, each 1-3 points: Bilirubin: less than 2 (1) · 2-3 (2) · greater than 3 (3) mg/dL Albumin: greater than 3.5 (1) · 2.8-3.5 (2) · less than 2.8 (3) g/dL INR: less than 1.7 (1) · 1.7-2.3 (2) · greater than 2.3 (3) Ascites: none (1) · mild (2) · mod-severe (3) Encephalopathy: none (1) · grade 1-2 (2) · grade 3-4 (3) Class A = 5-6 (well-compensated) Class B = 7-9 (functionally compromised) Class C = 10-15 (decompensated)
