Daily sedation interruption / target RASS 0 to -1

VTE prophylaxis (heparin/SCDs) unless contraindicated

H — Head of bed

≥30° to reduce VAP

U — Ulcer prophylaxis

PPI/H2 only if indicated (mech vent, coag, GIB hx) — de-escalate

G — Glucose control

Target 140-180 (NICE-SUGAR); avoid <110

B — Bowel regimen

Daily BM goal; senna/docusate prophylactic

I — Indwelling lines

Daily review — remove if not needed (CLABSI, CAUTI prevention)

D — De-escalate antibiotics

Re-evaluate spectrum + duration; cultures back?
